This issue is often caused by one of the following:
The image format is unreadable, that is, the image is either corrupt, saved in an obscure format or too large (a high DPI). Change the image format to one of either TIF, JPG or PDF, re-scan the form at 300 DPI in BLACK AND WHITE, NOT COLOR AND NOT GRAY SCALE, upload the new image and see if it works. You should also be able to test out the image you scanned by opening up the form file, click the recognition preview tab, click test preview, browse for the image and see if it detects.
There is some other error that the program did not expect. Please raise a support ticket as it is likely a bug. Please also attach both the form file, the segment file and the image that you scanned with the ticket.
